T Shirt Order Form Template - Does anyone know where i can get a general waiver for release of damages when we print on a shirt that a customer brings to us? Im looking for an embroidery work order form that contains images for shirts and caps front and back that i can edit and send to the printers. I've exhausted my search on the internet and haven't found anything appropriate. Is there any documentation that i should ask for to protect myself in the case that there is any confusion (in the future, from the gov., from the company, whoever.) about my legal right to print that logo? I would greatly appreciate it. This is not usually an issue if we provide the shirt because we can always order another but when they provide it it can be an issue.
